Resolved that the Minutes of the Social Care & Tackling Poverty Service Transformation Committee held on 24 November 2025 be approved and signed as a correct record.
Helen Williams, Principal Officer for Adolescent and Young People Services provided a “for information” report which presented an update on the development of our services in relation to child to adult transitions. The report brought together findings and insights from the initial work undertaken across both Child & Family Services and Adult Services in reviewing the transition process for young people moving into adulthood. The review had commenced due to the transition process being quite difficult and challenging to navigate for staff, young people and their families. It was hoped that the review would improve the process which sought to understand where improvements were required. Progress so far included: · Familiarisation sessions with staff in both Child & Family and Adult Services, Finance and regional colleagues; · Allocated project managers to provide peer oversight; · Data collection identified as requiring improvement; · Project Board established in order to develop a practice framework including practice guidance, identifying core principles around transition and identifying a clear transition pathway that was understandable and recognised as good practice. Stakeholder consultation would take place with young people and their families. The framework would be co-produced and embedded by March 2027. Governance and accountability for the project board would be shared by the Heads of Service for Child & Family Services and Adult Services. The Committee asked a number of questions that were responded to accordingly by the Principal Officer. The Chair thanked the Officer for providing the update.

Amy Hawkins, Head of Adult Services accompanied by Cathy Murray, Principal Officer for Internal Services and Deborah Webb, Strategic and Operations Manager for Day Services provided a “for information” report which outlined the agreed model to transform Day Services, progress to date and the next steps of implementation. The report provided an update to that provided last year and included activity undertaken to date and the next steps which built on the key outcomes of the review of Day Opportunities for Adults with a Learning Disability”, the review of the Older People’s Day Services and the review of Mental Health Day Services which had been proposed to Cabinet in March 2025. As a result, it had been agreed to align the existing day services to six types of Day Opportunities which reflected the needs currently being supported in our internal Day Services and via our commissioned day services. A commitment had been made to support the same number of individuals in the transformed new Day Opportunities Model that were previously supported by the internal day services (March 2025 totals, 522 people). The following had been agreed as Phase 1 of the implementation process: · Make temporary moves permanent and repurpose day service buildings not currently in use; · Identify capital works that may be required to ensure the longevity of the capital assets to ensure they were fit for purpose and the opportunity to align with grant funding; · Staff workshops on the scoping of and the next steps of the Internal Day Opportunities and, Service Assessments and reviews of existing commissioned services and bespoke arrangements purchasing Day Opportunities via Direct Payments. The Head of Adult Services then outlined the next steps of implementation of the new model which focussed on: · The finalisation of a commissioned Day Opportunities Framework; · Developing an overarching Communications Plan; · Development of the next phase of the Day Opportunities Transformation programme to be completed early 2026. In addition, the situation in relation to the Day Services buildings was outlined including commissioned condition reports and associated maintenance works to ensure they were fit for purpose. Decommissioned buildings were also listed. The Committee asked several questions which were responded to by the Officers. Councillor Baker raised concerns about limited access to services for elderly, disabled, and vulnerable individuals in her electoral ward. These issues would be picked up separately by the Cabinet Member Councillor Gibbard and the relevant Officers. The Chair thanked the Officers for the update.
The Chair presented the Work Plan for 2025-2026. Resolved that the Work Plan for 2025-2026 be noted subject to adding the following item to the 30 March 2026 meeting: “Social Care & Tackling Poverty Service Transformation Committee Annual Report 2025-2026”.
