Officers presented the Adult Services Performance Report for November 2025, covering activity across demand management, early help, reablement, commissioned care and safeguarding. Key Points Raised: • November saw a significant 22% drop in referrals to the Common Access Point compared to previous months. This mirrored the pattern seen in November 2024 and demand recovered in December. Officers emphasised that overall demand remains high. • Despite reduced referrals, early help performance remained strong, with 51% of contacts resolved with information and advice alone, and high proportions of cases closed without needing ongoing social care. • Officers reported improvements in the number of completed carers assessments but acknowledged continued work is needed to increase take‑up. Timing and repeated offers are important factors. • Residential reablement continued to perform well, with 98% occupancy and excellent outcomes. Home‑care reablement showed improved independence outcomes, with 53% leaving reablement independent, up from 34%. • Externally commissioned domiciliary care delivered around 81% of planned hours in October. Undelivered hours were attributed largely to unavoidable circumstances rather than provider issues • Safeguarding referrals remained high and increasingly complex. Officers noted this may require future resourcing considerations if the trend continues. Panel Members asked why referrals reduce in November; why carers decline carers assessments; what happens to cases after Home First involvement; what causes ‘undelivered hours’ in external care; what causes package of care delays and if safeguarding demand is sustainable.
Officers presented an update on the Adult Services Transformation & Improvement Programme, which supports the four‑year target operating model. Key areas of work included prevention, promoting independence, resource prioritisation, digital innovation and workforce development. Key Points Raised: • Growth in Early Help roles and improved coordination at the Common Access Point support earlier intervention and reduced escalation. • Strong reablement outcomes continue, with improved multidisciplinary working and reorganised internal home‑care reablement capacity. • Launch of the Smart Suite and the Ask Sara self‑assessment tool to help people explore equipment and technology that supports independence. Members expressed interest in visiting the Smart Suite. • Work is underway to recommission domiciliary care, develop community‑focused day services, utilise digital tools (Magic Notes), and progress the workforce strategy. • Officers confirmed close working with Health and Neath Port Talbot via the West Glamorgan Regional Programme, especially around hospital discharge pathways and assistive technology. Panel Members asked questions about how councillors should refer residents for support and how the service ensures Health and other partners stay informed about changes. Actions: • Arrange a visit to Smart Suite early in the new municipal year.
Panel considered the work plan and noted items for the next meeting.
Rhaid i gynghorwyr ddweud pan fydd ganddyn nhw gysylltiad personol â rhywbeth sy'n cael ei drafod, fel swydd, eiddo neu aelod o'r teulu. Mae buddiant sy'n rhagfarnu yn ddigon cryf i'r cynghorydd adael yr ystafell fel arfer a pheidio â chymryd rhan.
Caiff datganiadau eu darllen o'r cofnodion unwaith y byddan nhw wedi'u cyhoeddi.
Yn rhestru cynghorwyr presennol y cyngor yn unig. Nid yw aelodau cyfetholedig, swyddogion a chyn-gynghorwyr yn cael eu dangos.
