Resolved that the Minutes of the Social Care & Tackling Poverty Service Transformation Committee held on 13 October 2025 be approved and signed as a correct record.
The Director of Social Services provided a “For information” report to provide an overview of the target operating models for children and adult social services being developed through the social services transformation programmes to enable the committee to help inform and ensure wider understanding of the core purpose and intended end states of the Council’s Corporate Transformation Plan. The report was being presented to the Service Transformation Committee (STC) in response to the recommendation made in the Panel Performance Assessment (PPA) conducted under the Local Government and Elections (Wales) Act 2021. The PPA advised that the Council’s Corporate Transformation Plan (CTP) would benefit from greater integration and understanding across the Council and its citizens through more clearly articulating the purpose and end state of the transformation programme and what it intends to achieve for Swansea as a whole. The purposes of the report was to seek the STC’s input into defining the core purpose and end state of the relevant programmes within the CTP. This input would help ensure that each programme was aligned with the Council’s strategic objectives and communicated consistently across the organisation. The responses from the STC’s would be incorporated into draft definitions for approval by the Corporate Transformation Board in January 2026. The Director of Social Services explained that the currently established purpose of the social services transformation programmes was to ensure that the Council was able to provide sufficient, safe and financially sustainable models of care and support for individuals who have care and support needs or require to be safeguarded. The agreed target operating models for children and adult social services were the intended end states of the social services transformation programmes. These target operating models were predicated on the development of a range of safe and effective prevention and early help services designed to avoid or delay an individual requiring statutory managed care and support. Therefore, the intended end state of the social services transformation programme might be descried as right care, from the right person at the right time to ensure individuals, carers and families remain safeguarded and maintain their wellbeing. The Heads of Service for Child & Family Services, Adult Services and Enabling Communities outlined their individual Service transformation programme's vision, ambitions, and key projects in order to assist the committee in responding to the PPA. The Committee requested relevant written documentation be forwarded to them for further discussion at a workshop session in order that their recommendations be forwarded to the Director of Social Services prior to Christmas.
The Chair presented the Work Plan for 2025-2026. Resolved that the Work Plan for 2025-2026 be noted subject to the following amendments: 12 January 2026: Add – Day Services (Update) Slip – Tackling Poverty Strategy to 30 March 2026
Rhaid i gynghorwyr ddweud pan fydd ganddyn nhw gysylltiad personol â rhywbeth sy'n cael ei drafod, fel swydd, eiddo neu aelod o'r teulu. Mae buddiant sy'n rhagfarnu yn ddigon cryf i'r cynghorydd adael yr ystafell fel arfer a pheidio â chymryd rhan.
Caiff datganiadau eu darllen o'r cofnodion unwaith y byddan nhw wedi'u cyhoeddi.
