Matthew Brushett (Audit Wales) led the committee through the Audit Wales – Work programme and Timetable report. . It was confirmed that Andrew Mogford and CMT are included on that list.
Andrew Mogford led the committee through the Quarterly update on Corporate Risks – Quarter 1 report and advised that three risks were reduced, seven were unchanged and two had increased. He also confirmed that two emerging risks had been identified. In response to questions, Committee was advised that: · Follow up training sessions can be arranged for those members who were unable to attend · All risks are investigated and acted upon immediately, presented to the Governance and Audit Committee and formally added to the register following Cabinet approval.
Andrew Mogford introduced the report and highlighted a correction because the recommendation mentions (Appendix 1) and there is no appendix. All the information is included in the body of the report. It was confirmed that there are no major concerns. Historic management response forms were being addressed, and work was underway to develop a dashboard for improved transparency. A full list will be brought to a future meeting.
James Hopwood and Craig Flynn led the committee through the Preparation of Statement of Accounts – Progress Update report. It was acknowledged that the delay in the preparation of the 24/25 statement of accounts was a knock-on effect due to the 23/24 accounts not being submitted on time. It was confirmed that the draft statement of accounts was expected to be presented at the November Governance and Audit Committee, and this aligns with Audit Wales capacity. The Director of Finance confirmed that the focus is on quality over speed and that we are in line with other authorities in Tranche 2. In response to questions the committee was advised that: · Audit Wales has no current timeline in place for the submission of the statement of accounts for this year.
Andrew Wathan introduced the report to the committee and confirmed that four audit opinions have been issued, three reasonable and one limited. Further information was provided in relation to the limited opinion which related to Children’s Services Petty Cash, and it was confirmed that this would be revisited within 6 months and a further audit carried out which will be reported back to this committee. It was also confirmed that 30% of Audits carried forward into 2025-26 had been completed. In response to questions the committee was advised that: · There is still a need for petty cash in children’s services as it is used as pocket money for the children in care. The key aspect from an Internal Audit perspective is that all receipts are accounted for and submitted in a timely manner. · The limited report will be re-visited within 6-12 months, if there is a second limited or no assurance opinion it will be brought to the attention of the committee. · It was confirmed that the audit team is working well, and they have the resources to carry out the audit plan. It was also confirmed that the team has now been stable over the past 12 months, and the majority of the team have put themselves forward to participate in professional qualifications.
Joan Davies introduced the report and highlighted that only medium and high priority recommendations are monitored by the audit team, low priority recommendations are monitored by managers. Updates were provided on the six medium priority recommendations made since 31st July 2025 and the sixteen outstanding recommendations from previous audits (2023/24 and 2024/25). It was confirmed that the audit team regularly monitors recommendations and escalates undue delays or issues to the Councils Corporate Management Team and ultimately this committee. In response to questions the committee was advised that: · After an Audit has been completed, a management action plan is created with agreed actions and dates. If the recommendations are not implemented by an agreed date the responsible officer is contacted for an update. If implementation is delayed, a revised date may be set, reasons for this are usually due to factors outside their control such as system delays or staffing pressures. · It was confirmed that more information will be provided in futures reports to show whether the audits are on target / not on target alongside the report dates. · If an agreed date is pushed back more than once both dates are reported so that it is clear within the report and it is also reported to corporate management team.

The next scheduled meeting of the Governance and Audit Committee is 24th November 2025. .
The Chair and the Committee then reflected on the items that had been considered at the Meeting.
It was noted that the later start of 5pm had resulted in more members being able to stay for the whole of the meeting; but that if the change was introduced for all future dates, there may be difficulties with officer attendance. It was agreed that future meetings would be moved to 5pm and if there was an issue with officer availability for any of the scheduled dates, those meetings would be considered on an individual basis.
The Chair advised that there was no other business deemed urgent.
