The Scrutiny and Committee Services Officer presented Members with the individual Forward Work Programme (FWP) for the Social Care and Health Overview and Scrutiny Committee, together with the combined FWP. The following changes were highlighted: · The report relating to More Than Just Words, had been deferred until April 2023, as Officers did not have anything concrete to report on, other than they were putting together an action plan in respect of the national framework. · The annual report from the Joint Conwy and Denbighshire Adoption Panel had been deferred until April 2023 as the Interim Service Manager was new in post. · At present, there were no agenda items for the meeting scheduled for 15 March 2023, The Chair, Vice-Chair and Scrutiny and Committee Services Officer were keeping this under review in case any urgent reports came through. The Cabinet Member for Children, Families and Safeguarding, on behalf of the Committee, extended a warm welcome to Mr Robert Cain (Orthopaedic and Musculoskeletal Network Manager), who had been invited to provide Members with an update on Betsi Cadwaladr University Health Board’s (BCUHB) Orthopaedic Programme: Reference was made to the following: · The need for change:- - Pre-Covid BCUHB had up to a 2 year orthopaedic wait for elective procedures. - Post-Covid up to 5 years for an elective procedure. · Follow up waiting lists were increasing. · Service reviews: - 46 formal recommendations received in April 2022 (29 local to BCUHB, 17 national). - National Blueprint for Orthopaedic Surgery Delivery. · Orthopaedic initiatives including: escape pain classes, prehabilitation trial, high volume low complexity surgery, robotic survey and post-operative review list cleansing. · BCUHB short, medium and long term plans: -Short term: Maximise the use of Abergele, outsourcing and insourcing. - Medium Term: Ysbyty Gwynedd estates plan may lead to more bed capacity. - Long Term: Llandudno Orthopaedic Hub, which would see the development of 4 theatres and ward areas and allow for up to 4,800 procedures per year. This was currently in discussion with Welsh Government. - Long Term: Regional Treatment Centre for ambulatory planned care services was currently going through a full business case. Members welcomed the informative presentation and went on to discuss the following: • The BCUHB initiatives and plans (short, medium and long term) were welcomed, particularly maximising the use of Abergele Hospital, the Llandudno Orthopaedic Hub (in-patient Arthroplasty) and Regional Treatment Centre(s) for ambulatory planned care services. • A Member of the Committee expressed an interest in the BCUHB Central Area follow up list cleansing exercise, provided by one of BCUHB’s specialist physios and hoped a further update on the service could be provided in due course. In addition, an update on the National Exercise Referral Scheme, the cleansing exercise and any correlation was requested. • Demographics – the older population was increasing and there was a need, across all public services, to anticipate the significant increase in numbers and demands on services. • Operational pressures at Abergele Hospital and the need to use resources more efficiently was welcomed. In response, the Orthopaedic and Musculoskeletal Network Manager advised that there was evidence to show some operations could take place in procedure rooms as opposed to theatre rooms in order to free up capacity. • There were a host of ‘other’ services available to help patients manage their pain, whilst waiting for an operation/procedure. The need for improved social signposting to assist and direct patients was acknowledged. • Waiting lists and the need to share/communicate them wider via GPs, website, bulletin was highlighted. • Members fully supported the proposed Regional Treatment Centre(s) for ambulatory planned care services. It was acknowledged that the proposals were still in infancy/design stage. • Discussion arose regarding retention and recruitment. In response, the Orthopaedic and Musculoskeletal Network Manager advised that Treatment Centres would also become hubs for learning and be utilised by the medical school. • Lifestyle management was also acknowledged. Thanks was extended to Mr Robert Cain (Orthopaedic and Musculoskeletal Network Manager) and an invitation extended to provide a further update, to include the BCUHB Central Area follow up list cleansing exercise to the Social Care and Health Overview and Scrutiny Committee in 12 months’ time. RESOLVED: (a) That the update on the Betsi Cadwaladr University Health Board’s Orthopaedics Programme be noted. (b) That an invitation be extended to Betsi Cadwaladr University Health Board to provide a further update on the Orthopaedics Programme and Central Area follow up list cleansing exercise to the Social Care and Health Overview and Scrutiny Committee in 12 months’ time.

RESOLVED: (a) That the progress made by the Social Care and Education Service Development Programme be noted. (b) That a further progress report be presented to the Social Care and Health Overview and Scrutiny Committee in early 2024, to include an update on the roll out of the Older People Domiciliary Care Finance and Commissioning project.
The Cabinet Member for Integrated Adult and Community Services introduced the report and the Programme Manager, together with colleagues provided an update and summary of the progress made by the Social Care and Education Service Development Programme in delivering its objectives of progressing service changes/developments through a number of service change projects and tasks. The drivers for service changes were based on a mix of staff ideas, reacting to service challenges and pressures, responding to reviews, legislative changes and the ongoing financial situation the service and Council is facing. The current developments were varied, and focused on: · Additional Learning Needs transformation. · Service integration/closer working opportunities within the service. · Changing the way domiciliary care in Older People services was commissioned. · Developments within Children Services (focusing on supporting Looked After Children). New service developments to be progressed from 2023 included: · Transforming the way residential placements for children and young people in Conwy was delivered. · Increasing capability to deliver the Responsible Individual (RI) service standard regulations/duties (to promote, monitor and improve services) for all current, in the pipeline and for future developments involving in-house domiciliary care and residential provision. · The appendices to the report provided an overall summary on progress made on the 15 projects/tasks within the programme to improve the health and well-being outcomes for children, adults and families. In scrutinising the report, Members made reference to the following: Project 2.2.2 – Additional Learning Needs (ALN) Teaching Assistants Provision in Conwy: • ALN challenges, particularly post Covid. In response, Members were advised that schools across the County had seen a steep rise post pandemic, in ALN need, to include behavioural challenges and mental health concerns. The new Teaching Assistant Service provision model, which provided full funding delegation to schools without a Service Level Agreement, was now in place and it was hoped that the new sustainable service model would provide equitable and effective support for children and young people with ALN in Conwy. Project 2.2.3 – Older People Domiciliary Care Finance and Commissioning: • Discussion ensued regarding the importance of providing ‘outcome-focused’ care and provider commissioning improvements. • Concern was also expressed regarding the early termination of care packages. • The need to closely monitor the roll-out and progress of the project, reflect on service changes/lessons learnt and provide feedback to Members was highlighted. In response, Members were informed that the intention was, over time, to see an improvement and provide stability for the Domiciliary Care sector, to include, guaranteed core hours, flexibility and receive a standardised fee and set unit cost. In addition, it was hoped that early engagement with the Community Resource Teams would help prevent the early withdrawal of care packages. Members were also assured that the project and roll out would be closely monitored via the approved programme and project management framework and that a further update report reflecting on the changes, progress and lessons learned could be incorporated into the next update report to scrutiny. Benefits achieved through changes in the Social Inclusion Service: • An update/reflection on the project was provided by the Social Inclusion Service Manager. It was noted that, GCSE outcomes/data was not currently published as a consequence of the pandemic. Benefits achieved through changes in the delivery of Flying Start and Families First funding, leading to the creation of the Conwy Family Centre model: The Service Manager – Community Wellbeing provided an update and reflection on the project. Discussion arose regarding the following: • The service and its’s benefits were praised, particularly to the East of the County. • The need for effective delivery to the West of the County was highlighted. A Local Member felt that the current premises, together with communication/consultation and making use of wider knowledge and experience within the community was inadequate. In response, the Service Manager – Community Wellbeing advised that work in relation to the delivery of services, including accessibility along the West coastal belt was on-going.

RESOLVED: That the Social Care and Health Overview and Scrutiny Committee note the report. RECOMMENDED TO CABINET: That Cabinet approve the Social Services Representations, Complaints and Compliments Annual Report for 2021/2022.
The Cabinet Member for Integrated Adult and Community Services introduced the report, and the Team Manager – Quality Standards and Commissioning provided an overview of the statistical analysis of the representations, complaints and compliments received by the Conwy County Borough Council’s (CCBC) Social Services Department during the reporting period 1 April 2021 to 31 March 2022. The following key points within the report were highlighted: · Statistical and financial analysis in regards to the complaints received throughout the year (2021/2022). · Data in regards to the number and types of compliments received by the Department. · Lessons learnt and identified improvements. · The key aims and objectives for the Complaints Department for the forthcoming year (2022/2023). · The number of formal complaints received by the Department in the reporting year 2021/2022, was a total of 17 (15 at Stage 1 and 2 at Stage 2 of the process). · The timescales for resolving formal complaints had seen a slight decline. 60% of the Stage 1 complaints were completed within 15 days, in comparison to 78.6% in 2020/2021. In relation to Stage 2 Independent Investigations, 0% were completed within 25 working days, in comparison to 50% in 2020/2021. However, there was only one Stage 2 Investigation in 2020/2021 and only 2 in 2021/2022. It was noted that both Stage 2 investigations had started just before Christmas, which impacted on the timescales of both complaints, as staff were not available for interview due to annual leave. The Stage 2 investigations were both investigated remotely (due to Covid-19 restrictions) and there were some IT difficulties encountered on one of the complaints, which again caused some further delays, which impacted on the timescales. The correct process was followed in terms of seeking approval for the delays from the Director of Social Services, as well as informing the complainants, who were understanding. · There had been an increase in relation to the expenditure on Stage 2 complaints, £5,426 for 2021/2022, compared with £4,710 for 2020/2021. · Whilst there were only 2 Stage 2 Independent Investigations undertaken in 2021/2022, the expenditure included the cost of an Independent Investigation, which was completed in 2020/2021, however it wasn’t reconciled until the following financial year. · Lessons learnt – findings from the complaints process had led to identified actions and ultimately improvements in service delivery, for the Social Services Department as a whole, including the Complaints Department itself. · Members were informed that the identified improvements would help the Department deliver better outcomes and services to the local people of Conwy County. In scrutinising the report, discussion arose regarding the following: · The timescales resolving formal complaints was noted. · Trends and benchmarking with other Local Authorities. · The need for clear signposting to assist individuals through the complaints process. · Continual learning – the importance of lessons learnt continuing throughout the process was highlighted. Members supported the Annual Report and data and extended their thanks to the staff for their hard work. RESOLVED: That the Social Care and Health Overview and Scrutiny Committee note the report. RECOMMENDED TO CABINET: That Cabinet approve the Social Services Representations, Complaints and Compliments Annual Report for 2021/2022.
Councillor Kay Redhead declared an interest in Agenda Item 9i) – Progress Update - Social Care and Education Service Development Programme, as she worked for a care provider, which provided care to residents in Conwy. As the interest was not considered prejudicial, Councillor Redhead remained in the meeting and took part in the debate.
